A hair transplant can absolutely be worth the money and discomfort. But you need realistic expectations about what it can actually do.

_Three things I think anyone considering one needs to make peace with_

**You’re not getting your teenage density back.**

You only have so much donor hair to work with. If you’ve lost a significant amount of hair, especially around Norwood 4 to 7, there usually aren’t enough grafts to recreate the density you used to have.

A good transplant is largely about creating the _appearance_ of fullness through careful placement. That can still make a huge difference, but it’s not the same as restoring every hair you lost.

**Your old hairline probably shouldn’t be the goal.**

It’s tempting to ask for a low hairline, particularly when you’re younger. The problem is that it uses up a lot of grafts, leaving fewer available for the mid-scalp, crown, or another procedure down the road.

Donor hair is a limited resource. A slightly higher, age appropriate hairline is often a better long term choice than trying to recreate the one you had at 20.

**A transplant doesn’t stop hair loss.**

This is probably the biggest thing people underestimate. Surgery moves hair from the donor area to the thinning or bald areas. It doesn’t stop your existing non transplanted hair from continuing to thin.

If that happens you can end up with gaps behind the transplanted hair.

Treatments Finasteride Dutasteride Minoxidil may help maintain your existing hair, so it’s worth discussing with doctor.

Basically, a transplant is part of a long term hair loss plan, not a one and done reset.

If you’re comfortable with less density than you had as a teenager, a sensible hairline, ongoing medical treatments, you’re going into it with a much healthier set of expectations.
